Prior to surgery, please provide us with a copy of your insurance card as well as all other information necessary to file your claim. As a convenience to our patients, we will file your insurance claim and assist you with any follow up correspondence. The Surgery Center’s charge for your procedure is already established and available upon request. The charge does not include your doctor’s fee, anesthesia fee, or pathology studies. We accept MasterCard, Visa, American Express and Discover, as well as personal checks.

Workers’ Compensation
If you were injured at work and your employer is responsible for payment, your must provide us with documentation that your employer has accepted responsibility. You will need to furnish our business office with the name and address of your employer and workers’ compensation carrier for billing purposes. If your employer is unwilling to accept responsibility for the payment of your surgery or there has not been a report of injury filed with the Department of Labor, you will be expected to pay for the services.
